Breast Enlargement / Augmentation

This is the most popular cosmetic surgery procedure for women.  It is undertaken normally for a variety reasons as follows:

  1. Patients present with very small breasts which are not in proportion to the rest of their body shape
  2. Patient present to us when they have had children and the breast size and shape has changed following pregnancy.
  3. Patients present to us when they have had excessive weight loss and the breasts have started to sag and they are not as pert.
  4. Patients have different sized breasts e.g. one a lot smaller than the other
Patients who want to go bigger even already having breast augmentation in the past.

Procedures

This is normally undertaken as a general anaesthetic as on overnight stay.  Some patients can leave the same day if the surgery is performed early in the morning.  It takes approximately 1 – ½ hours to perform in Health Care Commission approved hospital. 

Types of Implants

There are many different shapes and types of breast implants and it is very important to fully discuss this with Specialist Breast Surgeon chosen as each individual person will require a different shape or type to achieve the desired look.  The most common type of implant used is a silicone round shaped implant but there is also a tear dropped shaped implant which gives a much natural look.  These will be discussed at A New You’s initial consultation thoroughly to ensure that you choose the right implant for your needs.

Placement of Implants & Scar

There are number of different ways to insert the implant.  The three most popular are:

  1. Inframamory. - in the natural crease underneath the breast.
  2. Areloa – around the dark area that surrounds the nipple.
  3. Axilla - under the armpit.

The method of inserting the implant below or above the breast muscle will be discussed in the consultation so that you will be given the most natural look and shape that you desire.

Scar

The incision is made in one of the above places.  It consists of normally small 5 cm incision which should over time heal from a red line to a white inconspicuous scar. If you are slow to heal there is special tape, gels or injections that can improve the scar if necessary.  Everyone heals differently but any scar is at its optimum at approximately 9 to 12 months.

Possible Side Effects & Risks

Bruising & swelling – There is bruising and swelling following this procedure which in the first instance is contained with a bandage which is secured around the breasts immediately post operatively.  This is removed before leaving the hospital.  Arnica tablets and cream are encouraged to be used post operatively and it is advisable in the immediate week following the operation to be propped up with pillows to help with swelling and fluid collection.  Very often one breast maybe more swollen than the other.  This is quite normal but any excessive swelling on either one side or both should be reported to the clinic.  

Bleeding (haemotoma) – this is a small collection of blood that forms near the implant and can show itself as a swelling and is visible.  This is normally avoided by the insertion of drains but it is not always necessary and it will depend on the Surgeon whether little drains are inserted or not.  If this occurs you need to contact A New You clinic so the appropriate action can be taken.  It is not that common and is easily rectified once it has been noticed.

Hardening (capsular contraction) – this has been greatly reduced by the newer types of implants that  have been available over the last 10 years.  Hardening used to be as high as 25% with the old implants but with the new implants which have a textured coating surrounding the silicone shell it has been reduced to 2 – 3%.  This is where the breast goes hard and your own body rejects the foreign material  that has been inserted.  There are degrees of hardening and some people like the effects of being slightly firmer.  However if the breast becomes painful or extremely hard then the implants need to be removed and replaced.

Infection

This is a rare problem and can present itself as swelling, redness and discomfort at the wound site.  This can easily rectified by a course of anti biotics.  However in some instances the infection can be internal around the implant.  If this occurs the implant needs to be removed and a course of antibiotics will be prescribed.  The implants will be replaced after a period of 12 weeks once the infection has subsided.

Recovery

You will have dissolvable stitches in and can normally return to work within 1 – 2 weeks.  It is important to wear a support bra immediately after the operation for a duration of 6 weeks in order to let the new shape and the implants settle. No strenuous physical activity should be undertaken for up to 4 - 6 weeks.  You will be advised what activities you can perform at the time of your check ups which will be undertaken at 1 and 2 weeks, 3, 6 and 9 months to monitor your recovery and the outcome of your surgery.
